How to Enroll

You are eligible if you are disabled or 65 and older, enrolled in Medicare Parts A and B, a Wisconsin resident and live within Health Tradition Health Plan’s service area. The Health Tradition Health Plan service area by county includes: La Crosse, Crawford, Monroe, Buffalo, Jackson, Trempealeau, Juneau, and Vernon counties.
