your Basic Medicare coverage. You will keep your regular Medicare card.

physicians and specialists at Franciscan Skemp, and to Mayo Clinic with a plan referral
for services not available locally.
- Guaranteed renewable for life. Your Health Tradition 65Plus policy is guaranteed
and cannot be cancelled. We will renew your policy for as long as you meet eligibility
requirements and make the premium payments.
How to Enroll
You are eligible if you are disabled or 65 and older, enrolled in Medicare Parts A and B,
a Wisconsin resident and live within Health Tradition Health Plan’s service area. The Health
Tradition Health Plan service area by county includes: La Crosse, Crawford, Monroe, Buffalo,
Jackson, Trempealeau, Juneau, and Vernon counties.
