Health Tradition Health Plan is a managed care organization that offers access to Mayo Cinic Health System providers in 12 western Wisconsin counties and several locations in southeast Minnesota and northeast Iowa. This provider network includes 17 hospitals, more than 800 providers and more than 58,000 pharmacies. Our providers for Premier and Premier Plus, Premier One, BadgerCare Plus and 65Plus are located in the towns indicated with a diamond, square or circle on their respective maps.
