Health Tradition Now Requires All Members to Select a Primary Care Provider
Effective August 1, 2010, Health Tradition Health Plan is going to require that all
members choose a Primary Care Provider (PCP). Having a PCP helps to ensure that you are
receiving the most efficient, and best care possible!
PCP’s are health care professionals who provide primary medical services and act as
your main contact with the health care system. Their job is to coordinate your care within
the system and with other specialists as needed. Examples of PCPs include: physicians,
physician assistants, or nurse practitioners in departments like family medicine, pediatrics,
internal medicine, OB/GYN and the Center for Women’s Health.
Please note that by selecting a PCP, it does not limit you from receiving health care services
from other Health Tradition participating providers and will not affect your benefits or coverage
through Health Tradition. Health Tradition Health Plan will be mailing Primary Care Provider Selection
Forms to all subscribers beginning in August. Please do not disregard this mailing! These forms must be
filled out and returned to Health Tradition Health Plan.
