What you’ll need when you go to HealthCare.gov
HealthCare.gov will lead you through a series of questions to help you find a health plan.
- Have your annual income information ready if you think you may qualify for a subsidy to help cover the cost of health insurance or a program such as CHIP or Medicaid
- Have your social security number handy, and numbers for any family members who would be on your health plan
- Know the birth dates of each family member who would be on the plan

Counselors are available at four Mayo Clinic Health System sites to help you select a plan. These counselors are specially trained to answer questions on the health exchanges and assist you in applying for coverage. Ways to contact them:

Local resources may exist in your community. One of these is Western Wisconsin Enrollment Network http://www.lacrossecounty.org/humanservices/aca, serving Clark, Buffalo, Trempealeau, Pepin, La Crosse, Monroe, Jackson and Vernon counties. Your county health and human services offices may have information for your area.

Download these applications to print out paper copies and submit by mail instead of applying online.
Application for Health Coverage & Help Paying Cost – Individual to apply for health coverage through the Marketplace at HealthCare.gov by mail. This form allows you to apply for subsidies that may pay a portion of the cost.
Application for Health Coverage & Help Paying Costs – Family This is the family version of the above application to apply for health coverage through the Marketplace at HealthCare.gov by mail. You may qualify for a free or low-cost program even if you earn as much as $94,000 a year (for a family of 4)
Application for Health Coverage – Individual Use this form if you would like to purchase health insurance but do not want to apply for subsidies that may pay a portion of the cost.
Small Business Health Options Program (SHOP) Health coverage application for employees Use this form if you would like health insurance through your employer. You may be eligible to purchase insurance through the Marketplace if you do not have access to an employer plan or if insurance currently offered by your employer does not meet certain expense and coverage criteria. The completed form should be submitted to your employer.
Small Business Health Options Program (SHOP) Health coverage application for employers If you have a small business and would like to begin offering insurance through this program, you may apply by mail. You will receive information on eligibility, coverage and cost comparisons and how to complete the enrollment process.
