PharmacistThe formulary published by Health Tradition Health Plan is an abridged version including the most commonly used drugs in the outpatient environment. The Health Tradition Pharmacy & Therapeutics Committee continually monitors current and new drugs. Periodically, the formulary is reviewed and modified with any changes effective as of the date of revision to ensure members receive the highest quality and most cost-effective prescription drugs available.

Important Points to Consider:

  • Medications requiring prior authorization are shaded on the formulary
  • Oral contraceptive coverage may vary depending upon plan benefits per group coverage
  • Over-the-counter drugs are not covered by the health plan
  • Pharmacy coverage is not provided by Health Tradition for BadgerCare Plus members as of 2/1/08. Click here for helpful information.

The formulary is available on-line for those providers having access to the Mayo Clinic Health System - Franciscan Healthcare intranet. The Mayo Clinic Health System - Franciscan Healthcare formulary will note differences for Health Tradition.
