Mayo Clinic care in western Wisconsin, with Health Tradition Health Plan

Mayo Clinic care

Health care that’s personal

Mayo Clinic developed this model of care… an extended care team starting with your PCP, with access to any Mayo Clinic specialist and a better patient experience.

A relationship with the provider who knows you best

Health Tradition’s benefits are built around the care you receive from your Mayo Clinic Health System or other select primary care provider (PCP). This is the doctor, physician assistant or nurse practitioner you choose to lead your care team.

Your PCP will get to know you, your history, preferences and goals, and make room in the schedule to see you sooner. You always know who to call.

You’ll see your PCP for routine preventive care

  • Annual exam
  • Immunizations
  • Screening for health problems
  • Help to manage any chronic conditions you may have
  • Coordinate your treatment with specialists
  • Identify health risks and help you avoid them

For one-hour treatment of common conditions

Mayo Clinic Express Care Online makes it convenientyou don’t have to take time off from work or leave your home. Virtual consultations are only $49, waived if an office visit is necessary. Simply complete an online questionnaire about your symptoms—the same questions your own doctor would ask you in person—and specify a pharmacy. You’ll hear back from a Mayo Clinic provider in an hour or less.

If you need a specialist

If you develop a condition that requires specialty care, your PCP will bring specialists to your care team. Your PCP has unique access to the extraordinary resources of Mayo Clinic. Because Mayo Clinic Health System and Mayo Clinic care is team-based, your extended team may include some of the nation’s leading specialists.

After-hours care

A Mayo Clinic 24/7 Nurse Line registered nurse is always available to answer questions, address concerns and give you advice on what to do about illness or injury, including what kind of care to seek and how to care for yourself at home. The nurse will follow up with you or make an appointment with your PCP if you wish.
