Getting Better Post-Discharge
After you are discharged from the hospital, it can be stressful and difficult to return to your daily life and activities. In order to remain healthy, its important to fully understand your discharge care, including medication instructions and knowing who or when to reach out if you need help. According to the Centers for Disease Control and Prevention, nearly 20% of patients experience adverse event or side effect within three weeks of being discharged – and over 75% of those events could have been prevented.
Our post-discharge care management program helps you get better faster by monitoring your progress and providing you the support and resources when you need it most. Our clinically proven program combines the ease of automated mobile messages with a team of accessible and supportive Health Tradition Care Management staff to track and support your journey to improved health.
