Health Tradition members must live in one of following Wisconsin counties at least 245 days each year:
Brown, Calumet, Door, Fond du Lac, Green Lake, Jefferson, Kenosha, Kewaunee, Manitowoc, Marinette, Marquette, Menominee, Milwaukee, Oconto, Outagamie, Ozaukee, Racine, Shawano, Sheboygan, Walworth, Washington, Waukesha, Waupaca, Waushara, and Winnebago counties.
Adams, Ashland, Barron, Bayfield, Buffalo, Burnett, Chippewa, Crawford, Douglas, Dunn, Eau Claire, Grant, Jackson, Juneau, La Crosse, Monroe, Pepin, Pierce, Polk, Richland, Rusk, Sauk, St. Croix, Trempealeau, Vernon, and Washburn counties.

Eastern Wisconsin Service Areas
Health Tradition 65Plus members in the eastern part of the state will have the opportunity to choose their community-based PCP from Aurora Health Care and Ascension Health Systems, along with other select local providers.
Western Wisconsin Service Areas
Health Tradition 65Plus members in the western part of the state will have the opportunity to choose their community-based PCP from Mayo Clinic Health Systems, Esentia Health System and Health Partners, along with other select local providers.
