As a Health Tradition member, you have access to many resources, ways to communicate with us and details on your benefits, claims and history. Some are available in this section of the website.

  • See your claims history
  • Check on your eligibility and benefit balances
  • View Explanation of Benefits (EOB) statements
  • Access plan documents and important member materials
  • Check HRA claims and balances
  • Get pharmacy information

Misplace a form?
Go to the Online Service Center anytime to download and print a variety of forms that may be needed to process your claims. If you are unable to determine which form you need, call 1-608-781-9692 or toll-free 1-888-459-3020 Our friendly and helpful staff will assist you in selecting the correct form to complete the claim. Some of the forms are:

  • Accident (or Injury) Form
  • Dependent Verification Form
  • Request For Medicare Coverage Form
  • OptumRX Prescription Drug Reimbursement Form
  • Health Reimbursement Account (HRA) Claim Form

Login to the Health Tradition ManageWell wellness portal for wellness benefits such as the health assessment, complimentary copy of the Mayo Clinic Guide to Self-Care, searchable Mayo Clinic health information, activity trackers and more. (Employer group members access the Eat Well Move More rebate at ManageWell.)

If you don’t have access to a computer, you may take the health assessment by phone instead of online. We will ask all the same health questions, in language that is easy to understand. Please call 608-392-4256 to request your health assessment by phone.
